A substance use assessment is a structured evaluation conducted by a trained professional to understand a person’s use of alcohol, drugs, or other substances. It helps determine the level of risk, identify any co-occurring mental health concerns, and recommend appropriate treatment support.
To goal is to understand your patterns, how use affects your life and can support any requirements from court, probation/parole, or work requirements.

What to Expect
During your assessment, you’ll meet 1:1 with Toni Miller a licensed addiction counselor via telehealth in a safe, confidential setting. We’ll ask you questions about your substance use, health, and lifestyle to understand your unique situation. There are no wrong answers—our goal is to listen, understand, and provide guidance. At the end of the assessment, you’ll receive clear recommendations and next steps whether thats therapy, treatment programs, or additional support. and Toni will help coordinate care by submitting a referral as needed.
Each assessment considers:
All substances currently or previously used
The effects of substance use on your life
Barriers to treatment or recovery
Your specific needs and appropriate treatment recommendations
Following the assessment, referrals to suitable treatment resources will be provided as needed.
Please note: At this time, services are limited to assessment and referral only. Ongoing substance use treatment or counseling is not offered.
Fees
Substance Use Assessment (60 minutes)— $225
All services are offered on a self-pay basis (Card including credit card, debit, HSA, or FSA). Insurance is not accepted at this time. MCC can provide a superbill to submit to insurance since MCC is out of network.

Thank you for your referral! Please complete the contact form at the bottom including your name, company, reason for referral, the client’s name, phone number, and date of birth and MCC will reach out to schedule with the client!
Note: Every client has the right to refuse services, MCC will notify you of efforts to initiate services for the assessment.

Once you schedule, MCC will confirm the appointment and send you intake paperwork through the portal. This will include releases of information for any referral sources that need you to do an assessment, a release for an emergency contact in case it is needed, a fee agreement, informed consent, and a social history to help inform the assessment.

During the appointment, you will meet by telehealth through Sessions Health and get to know the clinician providing the assessment, this is your opportunity to share about your experiences and what led you to the assessment. It is entirely up to you how honest you want to be about what has been going on for you, and this will help the clinician get the most support for you.

Fees for the assessment are due at the time of service, if they are unable to be paid then a rescheduling will be needed. Please give 24 hours notice before cancellation to prevent a 50% fee for late cancellations or no shows.
Note: If fees are a struggle MCC can provide alternative options for assessment, sliding fee and insurance are not currently offered.
